All applications for a permit to sell fireworks shall:

1. Be made in writing accompanied by a permit fee of three hundred dollars ($300);

2. Set forth the proposed location of the fireworks stand;

3. Include for delivery to the city or county clerk insurance certificates evidencing public liability coverage in the amount of two hundred thousand/ four hundred thousand dollars ($200,000/$400,000); and property damage coverage in the amount of $2,000 designating the city or county as an additional insured;

4. Include for delivery to the city or county clerk insurance certificates evidencing products liability insurance in an amount in excess of one million dollars' ($1,000,000) coverage;

5. Include a statement that the applicant agrees to comply strictly with the terms of any permit granted and to furnish any additional information upon request;

6. Include a sales tax permit. (Ord. 98, amended, 1983)
